Certification Form Instructions

THE DEADLINE TO SUBMIT A CERTIFICATION FORM IS 11:59 P.M. EST ON FEBRUARY 27, 2025.


Considerations and Preparation:

  1. To be considered for eligibility for a Distribution Payment from the Centaurus Fair Fund, you must provide the information requested in this online Certification Form and submit a release through the Fund Administrator’s online claim system. Failure to submit the completed form through the Fund Administrator’s online claim system so that it is submitted and received by the February 27, 2025, Claims Bar Date set forth in the Plan may result in your claim being rejected and you being precluded from any recover from the Centaurus Fair Fund.

  2. Timely submission of your Certification Form does not guarantee you will be eligible for a Distribution Payment from the Centaurus Fair Fund or that you will be compensated for your claimed loss.

  3. Please fill out this Certification Form completely. After submission of the Certification Form, you may be requested to provide documentation to support your claim. Failure to completely fill out the Certification Form or timely provide all requested documentation will result in rejection of your claim.

To start your online claim form, you can find your Unique ID and PIN on the Notice that was sent to you. If you are unable to locate your Unique ID and PIN, please contact the Fund Administrator via email at info@CentaurusFairFund.com or by calling 866-675-2467. You will be asked to provide the Class Member’s full name and mailing address.
